insurance and billing

Upon your first visit to CVT Surgical Center, we will ask you to complete an insurance information form. Please have the name and mailing address of your insurance company available as well as your policy and group numbers. It would be extremely helpful if you bring your insurance or Medicare or Medicaid card with you. We will also need to know if your insurance requires pre-certification, a referral slip from your primary care physician, and second opinions for surgical procedures, If needed, we will assist you in obtaining pre-authorizations and second surgical opinions.

CVT Surgical Center and its physicians are Participating Providers in the Medicare program. We will file all necessary insurance claim forms, including those for Medicare supplemental (Med-i-Gap) policies. Please supply us with any employee claim forms which need to be filed.

Each month you will receive a statement showing your charges for medical services rendered and the status of your insurance claim. Because insurance coverage can vary from plan to plan, please understand that you are responsible for any amount not covered by your insurance. Payment plans can be arranged in special circumstances.
